At PNW Guardian, we believe that guardianship should only be pursued after exhausting every lesser restrictive option first. Our Mission is to take a holistic approach in advocating for the person to maintain as many rights as possible and participate in the decision-making process. We seek to improve the quality of life for the person and maintain as much independence as possible for the person while ensuring appropriate services are provided for them.
A modern approach
It’s essential that our clients receive quality advocacy, the least restrictive environment, and that decisions are made to honor each clients’ preferences and needs, utilizing person centered planning.
Our approach is modeled after the NGA Model Code of Ethics, NGA Bill of Rights, and the NGA Standards of Practice.
Less Restrictive Alternatives to Guardianship
Power of Attorney
“Power of Attorney Document” means a written document expressly granting legal authority to another named individual(s) or agent(s) to act on behalf of and to manage some or all financial matters in the name of the individual creating the power of attorney under the terms and conditions set forth in the document. Advanced Directives for Healthcare in Oregon
Name a person to make your healthcare decisions if you could not make them for yourself. This person is called your healthcare representative and they must agree to act in this role.
You may be able to obtain Advance Directive and Healthcare Representative appointment forms from your health care provider.
Representative Payee
A representative payee is a person or an organization appointed to receive Social Security, USDVA, Pension, and/or other benefits for anyone who is unable to manage or direct the management of his or her benefits. A payee’s main duties are to use the benefits to pay for the current and future needs of the beneficiary, and to properly save any benefits not needed to meet current needs. A payee must also keep records of expenses and file a report or accounting to how he or she used or saved the benefits.
Healthcare Representative
For an individual who has self-appointed a healthcare representative with an advance directive, the healthcare representative has healthcare decision-making authority as described in ORS 127.505 though 127.660
